The notched metal on the bottom of the bolt face? Is it not supposed to look like that because I just noticed tonight that mine also looks like that.
Has anyone found a cure for this issue?
I’m having same problems. Probably 500 rds down 2-3 fale to eject with different ammo.

Show me the barrel feedramp and what spring/buffer?
Stock spring/buffer
First step for me would be to polish the feed ramp. It could be the ammo is dragging and that looks dirty. Second, i would add a heavier buffer if that didnt work. 9mm guns are extremely finicky.
Extreme solution: send that barrel to Rudy at Macon Armory and get his guarantee to feed feedcone on it.
What buffer do you recommend? This is a stock Arv is it typical to have to add a heavier buffer? Ammo used was 115 PMC and some 115 reloads. Both have given me the issue.
New info…using reloads is a mixed bag because of your specific load. PMC is factory ammo but need to try another factory load.
Make sure you are not getting overtravel on the bcg before a buffer change You can do this by pulling back on the CH (no mag and no live ammo!) and then using your finger through the ejection port to continue pushing the bcg rearwards. If it continues to be pushed back, overtravel can lead to bolt bounce, double feeds, etc. If it does continue back, you need a buffer spacer.
Some folks use quarters (google it) or you can get a delrin spacer at Lowes/Home Depot for a buck or two (this is what i use).
Then try the gun with that. If you still have issues then change buffer weight.
I prefer KAK 9mm buffers. Weigh yours and see the weight, then go up. You can always add weights to your current buffer for cheaper.
With the ch all the way back and bcg all the way back it doesn’t seem to move back at all when I push on it

Another piece of the puzzle…is that buffer tube standard length or shorter?

That is a standard c tube…i would make your buffer heavier or get a heavier one. May also need a heavier spring also.
Did you buy this complete or as a separate upper and lower?
Complete. The link I sent was what I bought only modification was the red dot and handrail.
